There are two options for completing this cast-on.
Option 1
Step 1: Start in the slingshot position
Start in the slingshot position, just like you would for the Long Tail Cast-on.

Step 2: Touch the needle to your thumb
Touch your right needle to the base of your thumb.

Step 3: Slide under both strands
Slide the right needle under the two strands around your thumb.

Step 4: Bring needle up to thumb tip
Bring the right needle up to the tip of your thumb.

Step 5: Slide through the two thumb strands
Slide the needle down your thumb so it goes through the two strands around your thumb.

Step 6: Grab yarn from index finger
Bring the right needle over the strands around your thumb and grab the yarn around your index finger.

Step 7: Find the small triangle
Find the small triangle that is formed just above your right thumb.

Step 8: Pull yarn through the triangle
Using the right needle, bring the yarn from your index finger through the small triangle at the base of the needle.

Step 9: Reposition for next stitch
You have successfully cast on a stitch using the German Twisted Cast-on! To reposition for the next stitch, remove your thumb from the loop and reposition back to the slingshot.

Option 2
If you're having trouble seeing the tiny triangle to bring the index finger yarn through in step 7, this might be the better option for you.
Complete steps 1 through 6 the same as in Option 1.
Step 7: After you grab the yarn
After you grab the yarn wrapped around your index finger with your right needle...

Step 8: Bring your thumb to your index finger
Bring your thumb to the base of your index finger.

This will untwist the yarn that was wrapped around your thumb, opening it up so you don't have to look for that tiny opening at the base of your right needle.
Step 9: Pull yarn through the loop
Bring the index finger yarn wrapped around the right needle down through the loop around your thumb. And you have successfully casted on a stitch! Reposition the same way at the end of Option 1 so you are back in the slingshot position. Repeat the steps from Option 1 or 2 until you have the number of stitches you need on your right needle.
